Folic acid, a supplemental folate not found in nature, must be reduced during absorption to become metabolically useful. This transformation process is easily overwhelmed and unmetabolized folic acid can enter the bloodstream. Folic acid activation also depends on the enzyme methylenetetrahydrofolate reductase (MTHFR) for conversion to L-5-MTHF. Genetic variations of MTHFR activity affect up to 25% of the population, which can impair the ability to fully metabolize folic acid. Unmetabolized folic acid can inhibit uptake by cells of biologically active reduced folates and have other detrimental anti-folate effects. L-5-MTHF is the principal, active form of folate in blood and the only folate that can cross the blood-brain barrier. It is involved in neurodevelopment and mood regulation, and is the primary methyl donor for conversion of homocysteine to methionine.
