Example 1: if your travel dates is from 19th March to 26th March (8 days, as both dates are inclusive), pick the 1-9 days.
Example 2: if your travel dates is from 15th March to 26th March (12 days, as both dates are inclusive), pick the 10-12 days.

For Each Insured Person(s)
1) Name
2) NRIC/FIN/Passport Number 
3) Date of Birth
4) Relationship to Proposer if not buying for yourself

Travel Details
1) Date of Departure from Singapore & Date of Arrival in Singapore
2) Destination(s), Country(ies) to Visit
